Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: Operator was tending two Lull 8044 rough terrain forklifts that were out of view from each other. Four employees, two in each unit, were left unattended for durations throughout the work day at a working height of 21' to grade.

Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: Working platform baskets were attached to Lull 8044 rough terrain forklifts with link chain and open end hooks. A positive locking device was not supplied to prevent accidental forward motion of the basket.
