Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Employees were exposed to a fire and explosion hazard from not being adequately trained on appropriate procedures when cyclones developed hot spots. (b) Employees were exposed to a fire and explosion hazard from the snuff steam line being locked out and were not able to quickly activate the snuff steam to put out hot spots in the #4 cyclone. Among other methods, one feasible and acceptable method to correct these hazards would be to develop procedures to follow when cyclones develop hot spots and train personnel on these procedures. Among other methods, one feasible and acceptable method to correct this hazard would be to add ventilation to remove steam in the wet cake load out area.
