The other acetylene cylinder was stored on the west side of the powerhouse

outside the south overhead door.
C)Location:  Beneath boiler number 1, bays 3 and 4, Sithe Fore River
Powerhouse,
Weymouth, MA.
Three oxygen and two acetylene cylinders were not secured from tipping.
D)Location: Sithe Fore River Powerhouse, Weymouth, MA.
Oxygen and fuel gas cylinders were not seperated.
1)One propane and two oxygen cylinders stored together adjacent to the gas
compressor building.
2)Oxygen and acetylene stored together on a cart in the large warehouse,
Hingham
Shipyare pre fabrication area.
3)Oxygen and acetylene stored together next to the plumbers gang box,
Hingham
Shipyard pre fabrication area.
4)Two sets of oxygen and acetylene stored together on the low roof, west
side of
the powerhouse.
E)Location: Elevation 20'- 0", south end, behind Alimak elevator, Sithe
Fore River
Powerhouse, Weymouth, MA.
1)One oxygen cylinder was directly beneath a welder.
