Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to the hazard of being crushed by pipe piles if they rolled off the semi trailer. Location: St Botolph Street and Mass Avenue, Boston, MA An employee was struck and fatally injured when two 10 3/4 inch by 50 feet long pipe piles rolled off the truck while being rigged for off loading. Among other methods one acceptable method would be, prior to unloading pipe piles or similar materials, the loads should be thoroughly examined to determine a safe method of unloading, such as chalk blocks to keep the pipe piles from rolling.g,
