North Carolina General Statute 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish
employment and a place of employment which were free from
recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed
to:
a)fall to lower level in that the hoistway door on the
second level could be raised with the elevator at another
level.
Among other methods ,one feasible and acceptable abatement
method to correct this hazard is to replace/repair the
applicable interlock switch and to establish periodic
inspection/testing of the hoistway door to assure the proper
operation of the interlock.
