Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish to each of his employees employment and a
place of employment
which were free from recognized hazards that were causing or were likely
to cause death or
serious physical harm to employees in that employees were exposed to a
struck-by hazard:
On or about 01/15/02, at the south side of the grass test site, an
employee was exposed to a
struck-by hazard while cutting 42-foot high tree without having received
training on safe tree
cutting/removal operations.
Among other methods of abatement, a feasible and acceptable method to
correct this hazard is
to follow the safety requirements for tree care operations of the ANSI
Z133.1 standard.

a man basket:
On or about 01/15/02, at the south side of the grass test site, an
employee was exposed to a fall
hazard while working from a job-made work platform approximately 12 feet
from the ground
level without wearing fall arrest equipment.
Among other methods of abatement, a feasible and acceptable method to
correct this hazard is
to prohibit employees to work from unapproved accessories such as job-made
work platforms
not equipped with lanyard attachment points.

fall hazard:
On or about 01/15/02, at the south side of the grass test site, an
employee was exposed to a fall
hazard  while working approximately 12 feet from the ground level from a
job-made work
platform which was placed on a set of forks the employer attached to a
front-end loader.
Among other methods of abatement, a feasible and acceptable method to
correct this hazard is
to ensure any attached equipment is used according to manufacturer's
instructions.
