employment free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed being buried by cave in.

the highway were not protected by the Five Parts of a
Traffic Control Zone by installing cones and assigning a
flagman to control traffic and ensure the safety of the
employees.

ensure compliance with requirement on a regular basis.r
