North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to falls to lower elevations: a)An employee was observed working in a manufactured personnel platform, assisting the worker on the roof in installing backing for fascia, approximately 7'9" above the ground, while the platform was not securely attached to the forks or lifting carriage. [Ref: ASME B56.6-1992, Safety Standard for Rough Terrain Forklift Trucks, paragraph 5.15.1(b)] Among other means, one feasible and useful abatement method be to use a chain and attach the personnel platform to the forklift lifting carriage.

Among other means, one feasible and useful abatement method be to use the referenced standard in constructing the personnel platform.
