Section 5(a)(1) of Public Law 91-596, the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to being caught between and/or crushed by shifting or collapsing units weighing from a few tons to over a hundred tons: (a)Production Area, Bay 2 - Employees were exposed to being caught between and/or crushed by a shifting or collapsing unit weighing several tons which was not adequately supported by the angle pin jig and bottle jacks (Simplex Superjacks). Feasible and acceptable abatement method to correct this hazard is to: 1.Position the unit on the angle pin jig so the load is adequately supported to prevent shifting and/or collapse: 2.Ffollow the Operating and Maintenance Instruction for Simplex Superjacks, Instruction Sheet 54216, which requires: a. Never exceed the rated capacity of the jack, b. Place the load on a firm foundation, c. Follow the load with blocking; and d. Prior to each use and when returned to the tool room, each jack's external condition shall be visually inspected and the operating condition of each jack shall be checked for proper operation. Any jack found to be not operating properly shall be taken out of service and shall not be put back in service until a competent tool room service person has repaired and certified the jack.
