Section 5(a)(1) of Public Law 91-596, the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to: Hazard of being struck by and/or crushed by materials dropped from defective below the hook lifting devices: (a)Foundry building - Spreader bar on 5 ton P & H underhung single girdle top running gantry crane was not marked with rated load. (b)Foundry loose pattern area - Spreader beam did not have an identification certifying the lifting capacity. (c)Loop bed molding area - Wire rope sling attached to spreader beam were attached with single wire rope U-bolt clip. (d)Grinding department, west side station #1 - Home made lifting prong(s) were used for blow bar(s) # 1044 and #1053. One feasible and useful abatement method to correct this hazard is to: (1) Follow the requirements of American National Standard, ANSI/ASME B30.20 - 1993 for Below The Hook Lifting Devices by posting the rated load capacity, installing the correct number of U-bolt clips and removing home made lifting prongs.
