Labor and Employment Article, Section 5-104(a):  The employer did not
furnish employment
and a place of employment which were free from recognized hazards that
were causing or
likely to cause death or serious physical harm to employees:
(a)Date of Violation:On or about 03/08/01
Location:Wet Mix Room
Condition:Employees removed combustible flare ignition mixture residues
from
the floor and other surfaces using a ferrous metal floor scraper.
Among other, one feasible and acceptable abatement method to correct this
hazard is to
prohibit the use of any potential ignition producing scraping tools and to
fully cover the floor
using disposable coverings to minimize manual cleaning.

Location:Button Fill Room

Condition:Building surfaces and equipment had not been vacuumed nor had
other
effective dust reduction means been taken to minimize the residual dust
from the "flare ignition mixture" prior to employees using compressed
air, at approximately 90 p.s.i. from a handheld nozzle, to "blowdown"
the room during cleaning operations.
Among others, one feasible and acceptable abatement method to correct this
hazard is to
follow the requirements of NFPA 654:
vacuum the area and equipment prior to blowdown; shut down or remove
electrical power
and other ignition sources; use only low pressure [15 p.s.i. (103kPa)]
steam or compressed
air to clean; ensure there are no hot surfaces in the area being cleaned
capable of igniting a
dust cloud or layer.
