Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following: As of and prior to February 11, 2004, Pot Room Utility-Drag employees were exposed to flying molten metal, struck by failed equipment (clamps, hooks, other parts), and in extreme upset conditions, electric shock during bridge raising duties. a. The employer failed to follow their own Safe Efficiency Procedure for bridgeraising (SEP 418-30) in that employees routinely raise the bridges without having all of the gang clamps installed, or without using the specified single or double anode clamps where gang clamps are unable to be used to secure the anode rods to the superstructure. b. The written bridgeraising procedure did not specify what actions employees must take in situations for "putting out a light," such as, but not limited to, making adjustments to the anode clamps, anode rods and carbon blocks. The procedure does not specify the "special precautionary measures" to be taken by the supervisor, if anode rods can not be secured before bridgeraising. Means of abatement, may include, but are not limited to, the following: (1)Procure, or develop, a single and double anode clamp; (2)Expand the SEP to include all duties necessary for safely raising the bridge, including actions employees must take to correct and prevent upset conditions, such as "putting out a light," and "dropping carbon." (3)Fully explain the "special precautionary measures" in the SEP for supervisors and how to accomplish them. (4)Train all effected employees (hourly and supervisory) on the expanded procedures and the potential hazards related bridgeraising. (5)Follow the procedures in the revised SEP 418-30 and require the use of the procedure by all employees. (6)Implement the use of effective single and double anode clamps.
