North Carolina General Statute 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish to
each of his employees conditions of employment and a place of
employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees:
a)press room, abrasive cut-off wheels were not
rated for the Milwaukee 3" straight grinder;
the spindle speed of the grinder exceeded the
maximum operating speed marked on the wheels,
which contributed to an employee accident on
March 24, 2001.
b)press room, employees utilized improper
abrasive wheels which were not approved for
use on the Milwaukee 3" straight grinder and
for the process which the employees were
utilizing the wheels.
NOTE:Among other methods, one feasible and
acceptable method to abate this hazard is to
consult with the manufacturer's
recommendations for the approved use of
specific types of abrasive wheels.  Another
method is utilize the operator's manual and
manufacturer's recommendations on the
Milwaukee straight grinder.
