Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazed of falling 25-30 feet from a job made
platform box.
a)700 Bedford Road -South side -  Two employees were 25-30 feet above
ground installing metal sheeting to a building using a job made steel box
that
was placed on the forks of a rough terrain forklift without a personal
fall arrest
system. The job made box was not secured to prevent the job made box from
falling off of the Lull Highlander rough terrain forklift model # 844C and
the
personnel inside the job made box were not protected from falling out of
the
box by the use of a proper guardrail system or personal fall arrest system,
observed on or about 4/19/02.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THE HAZARD IS:
(1) Follow the safety procedures in ANSI B56.6-1992, Para. 5.15 items (a)
- (y) American
National Standard for Rough Terrain Forklift Trucks.
(2) Provide a standard personnel lift equipped with the required safety
features.
(3) Follow the required safety guidelines for fall protection in 29 CFR
1926.502
NOTE: Because abatement of this violation is already documented in the
inspection case
file, the employer need not submit certification or documentation of
abatement of this
violation as normally required by 29 CFR 1903.19.
