were causing or likely to cause death or serious physical harm to

(a) Production area - the vertical band saw was not equipped with a
magnetic
restarter or similar device, as prescribed by the American National
Standard for
Metal Cutting Saws, ANSI B11.10-1990, that in the event of an interruption
in
the electrical supply, would prevent the saw from restarting automatically
when
power is restored.
(b) Production area - the vertical band saw and the Lockformer band saw
were
equipped with toggle switch-type, motor starters instead of motor starters
located,
designed, constructed, interlocked, or guarded to protect against
unintended
operation, as prescribed by the American National Standard for Metal
Cutting
Saws, ANSI B11.10-1990.
Abatement certification only is required for this item in accordance with
the
requirements of 29 CFR 1903.19(c).
