Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the following condition(s): Struck-by Hazard
a) An employee was exposed to the hazard of being struck by falling
concrete
mattresses/pads when being lifted by Unit 500 Pedestal Crane located at
the site Submar,
Inc., & 805 Dunn Street, Houma, La - The employer did not perform complete
Periodic/Annual inspection of the crane.  The employer did not make a
determination as
to whether deficiencies constituted a safety hazard.  Among other methods,
one feasible
and acceptable abatement method to correct this hazard is to implement a
regular monthly
program as required by the American Society of Mechanical Engineers (ASME)
Standard
ASME B30.4-1996, Chapter 4-2 Inspection, Testing, and Maintenance; Section
4-2-1,
Inspection General; Sub-Section 4-2.1.2 Inspection Classification a and b;
Section 4-2.1.4
Periodic Inspection; Section 4-2.2: Testing, and Section 4-2.3:
Maintenance.
