Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
On and before 10/23/01, employees were exposed to serious injury from two
inadequately
banded steel coils located in the Coil Bay at the south-east corner of the
building.
Among others, the following are feasible and acceptable methods to
minimize the probability of
injury during banding and coil handling operations:
1. Develop safe banding procedures which list the sequence of banding
operations.
2. Train all affected employees  on the requirements set forth in the
procedures and related
safety precautions.
3. Enforce employee use of the procedures.
The following are ANSI B11.18-1992 recommends for the proper banding:
- Banding material should me a minimum of 1 1/4 inch wide x .031 inch
thick, heat treated.
- All banding seals should be double crimped.
- corner protectors should be used on all bands placed through the eye of
the coil. They should
be sufficient enough to protect against rupture of the band due to sharp
edges of the material or
too acute bend.
Bands should be placed on the circumference of the coil as follows:
Place a minimum of on band (centered) on coils up to 12 inches in width;
for coils over 12
inches in width, place a minimum of two bands (equally spaced) around the
circumference.
