employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to:
On and prior to 12/11/01, employees were subjected to the hazards of being
crushed and/or caught between
vessels that were placed on rollers, resulting in fractures, multiple
internal injuries, or death.  The employer
then, without the manufacturer's approval or consent, welded steel plates
up against the rollers in an attempt
to prevent the bearings from popping out from being overloaded.
Among other methods, one feasible and acceptable method to correct the
hazard is to contact the manufacturer
of the rollers to get the right size bearings to hold the load that is
imposed upon them.
