North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)facility; manual turning area - monorail system has not been load tested. b)facility; roll turning area - monorail system has not been load tested. REFERENCE: ANSI B30.11-1998; 11-2.2.2 Load Test (a) New, reinstalled, altered, repaired, and modified equipment should be load tested prior to initial use... NOTE: One acceptable and feasible means of abatement is to have the monorail systems load tested.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)facility; roll turning area - there was no stop to prevent hoists contacting each other on monorail system. REFERENCE: ANSI B30.11-1998; 11-1.3.1 General Construction (g) Stops shall be provided at the ends of the carrier or crane travel to prevent the carrier or crane from inadvertenly coming off the track or contacting an obstruction. NOTE: One acceptable and feasible means of abatement is to install a stop between the hoists.
