North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)in the maintenance shop, 3-Ton, Yale, Spur Geared Block hand chain operated chain hoist mmoved from another shop had not been load tested. Ref. ANSI B30.16-2.1.1(a) Note: One method to abate this condition is to have the system load tested after the move to the new location.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)woven area, 2 Yale Spreader Bars were not clearly marked with the rated load. Ref: ASME B30.20.1.2, Below-the-Hook Lifting Devices, Markings and Construction. Note: One method to abate this condition is to have both spreader bars clearly marked with the rated load. b)woven area, 2 Yale Spreader Bars were not inspected. Ref: ASME B30.20.1.3(b) (b)Inspection procedures for lifter in regular service is divided into two general classifications based on intervals at which inspections should be performed. The two general classifications are frequent and periodic: (1)Frequent Inspection. Heavy Service - weekly to monthly (2)Periodic Inspection. A visual inspection conducted by an appointed person who makes records of apparent external conditions......An external code mark on the lifter is an acceptable identification in lieu of records. Heavy Service - monthly unless external conditions indicate that disassembly should be done to permit detailed inspection - semiannually. One recommended method of abatement is to appoint a designated person to start a program of scheduled maintenance, i.e., inspections.
