Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to laceration/amputation hazards:
(a) Shop Area-  Provisions were not made to prevent industrial machinery,
including but not
limited to, the Toledo and the Jet vertical band saws from automatically
restarting upon
restoration of power after a power source failure; and the motor starters
were not protected
against unexpected or accidental activation (had toggle switches).
Among others, one method of abatement is to equip the machinery with an
electrical device
that is designed to prevent automatic restart after power has been
restored to require
operating levels; and to install and utilize a starter controls that would
prevent unexpected or
accidental activation, as prescribed in NFPA-79, Electrical Standard for
Industrial
Machinery, Section 8.
Abatement documentation is required for this item in accordance with 29
CFR 1903.19
paragraph (d).
