Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer failed to provide a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees: a. On or about June 4, 2001, at the jobsite located at 434 West Roscoe Street in Chicago, Illinois, employees working on a two-point suspended scaffold were exposed to injuries from falls due to improper use of outrigger beams. The proper usage of the scaffold outriggers requires a 1000 lb counterweight to be used on each outrigger beam with a 36 inch overhang. This information is provided by the manufacturer in the manufacturer instructions. No abatement certification or documentation is required for this item.
