Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following conditions: a)Located at the Clute Diehl #2 well site, an employee was exposed to struck-by hazards due to an open-ended flow line that was not secured as required by Section 12.4.3 of API 54-1999. Among others, one feasible and acceptable method to correct the hazard is to properly secure the flow line to the ground. *** Certification and Documentation of Abatement Required ***
