Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the company did not ensure jib cranes were inspected frequently and periodically for safe operation: (a)Wyman Gordon Forgings, 10825 Telge Road, Houston, TX, 77095, Building 10, were exposed to struck by and/or electrical shock hazards because jib cranes, including, but not limited to: 1.Jib crane, Appl# 00154, located outside 35K Ton Press Area - in compressed gas cylinder storage area - hoist chain corroded and mechanical and electrical parts exposed to weather conditions. 2.Jib crane, Appl# 00162, located in Electricians/NC Tech Area, pendant cord separating from hoist assembly. 3.Jib crane, Appl# 00204, located in Inspection Area, boom drift. One feasible and useful abatement method to correct this hazard includes, but is not limited to: Conduct frequent and periodic inspections in accordance with ANSI B30.11 Monorail Systems and Underhung Cranes. *** Certification and Documentation of Abatement Required ***

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following conditions: (a)Wyman-Gordon Forgings, 10825 Telge Road, Houston, TX, 77095, employees were exposed to the hazard of being struck by and/or crushed by extruded pipe, at temperatures exceeding 800 degrees, because the OD gripper was not inspected frequently and periodically to insure that the teeth on the gripping wedges were dressed and kept smaller than 1/8", and that the sliding surfaces between the gripper wedges and housing were kept smooth, to prevent slippage of the extruded pipe in the OD gripper. Load testing documentation was not available. (b)Wyman-Gordon Forgings, 10825 Telge Road, Houston, TX, 77095, employees were exposed to the hazard of being struck by and/or crushed by pipe being transported using shop-made bar handling fixtures that were not inspected frequently and periodically, and one of which had not been load tested. (c)Wyman-Gordon Forgings, 10825 Telge Road, Houston, TX, 77095, employees were exposed to the hazard of being struck by and/or crushed by falling loads because L hooks were not inspected frequently or periodically. Die penetration testing done subsequently was not documented. (d)Below the hook lifting devices in the heat treat area comprised of a bar fixture with chain attachments were not labeled with the maximum lifting capacity of the device. Feasible and useful abatement methods to correct these hazards include, but are not limited to: (1) Load test and inspect all below the hook lifting devices in accordance with ANSI B 30.20 - 1993 Below the Hook Lifting Devices (2) Utilize recommendations of Engineering Company employed to analyze the gripping mechanism. *** Certification and Documentation of Abatement Required ***
