40.1-51.1(a) of the Code of Virginia:  Midwesco Filter Resources, Inc.,
did not furnish employment and a place of
employment which were free from recognized hazards that were causing or
likely to cause death or serious physical
harm to employees in that employees are operating two Harrington 1/2 Ton
Peerless electric chain hoists in the Lay-Cut and Slitter Areas, which
exposes them to:
(a)  Contusions by not having the latches securely attached to the hooks
of
both hoists.  The safety latches have been
taped back to the hooks preventing them from closing.  One feasible and
acceptable abatement method, among other
methods, would be to place new latches on the hooks such as that called
for by ANSI 30.16(b) 1990, American
National Standard Safety Code for Overhead Hoists.
(b) Electric shock by not having strain relief on the electrical cord to
the pendant control box of the hoist located in the
Lay-Cut Area.  The cord has broken away from the box exposing wires.  One
feasible and acceptable abatement
method, among other methods, would be to place a new electrical cord on
the pendent control box such as that called
for by ANSI 30.16(b) 1990, American National Standard Safety Code for
Overhead Hoists.
(c)  Contusions by not having the maximum load capacity posted on the
traveling beam for the hoist located in the Lay-Cut Area.  One feasible
and acceptable abatement method, among other methods, would be post "1/2
Ton" on the
beam such as that called for by ANSI 30.16(b) 1990, American National
Standard Safety Code for Overhead Hoists.
(d)  Contusions by not performing documented periodic inspections (annual)
for both hoists.  One feasible and
acceptable abatement method, among other methods, would be to conduct
annual inspections (documented) of all
components for both hoists, such as that called for by ANSI 30.16(b) 1990,
American National Standard Safety Code
for Overhead Hoists.d
