Carolina:  The employer did not furnish to each of his employees
conditions of employment and
a place of employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to employees in that employees were
exposed to:
a)lumber storage shed area - the Toyota forklift, s/n 2FG35-60324A, was not
equipped with a seat belt at the operator's station which exposed the
operator to
being caught/crushed under the forklift should the vehicle be up-set.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to install an approved seat belt at the operator's station.
