Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls to the ground: a.Lull Lift Platform - The platform did not have a guardrail system around its periphery or a personal fall arrest system. b.Lull Lift Platform - The platform was not securely attached to the lifting carriage. c.Lull Lift Platform - The maximum working load including personnel and equipment was not prominently indicated on the platform. d.Lull Lift Platform - The weight of the empty platform was not prominently indicated on the platform. e.Lull Lift - The truck was operated on footing which was not firm. ABATEMENT NOTE: One feasible and acceptable method of abatement would be to comply with requirements of the American Society of Mechanical Engineers document ASME/ANSI B56.6-1992, Safety Standard for Rough Terrain Forklift Trucks.
