Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls to the ground: a.Traverse Lift 6035 - The platform did not have a 4 inch toe plate. b.Traverse Lift 6035 - The platform did not have a plate with the following information on it: maximum work load including personnel and equipment; and weight of the empty platform. c.Traverse Lift 6035 - The platform was not secured to the vertical face of the forks, carraige or lifting mechanism. d.Traverse Lift 6035 - The platform did not have a means to securely attach the platform to the lifting platform to prevent the platform from pivoting. ABATEMENT NOTE: One feasible and acceptable method of abatement would be to comply with requirements of the American Society of Mechanical Engineers document ASME/ANSI B56.6-1992, Safety Standard for Rough Terrain Forklift Trucks.
