Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)burns when opening the main steam supply valve for the facility.  Among
other
means, one way to abate this hazard is to: (1) use the proper sized bolts
obtained
from the chart in, "The Pipe Fitters Blue Book" published in 1973 by the
Graves
Publishing Company, page 109, for a series 150 flange. The bolt for an
eight
inch, 150 #, carbon steel, raised face, weld neck flange should be at a
minimum
of 3/4" x 3 1/2".  (2) to establish a protocol for the installation and
inspections
of piping systems in accordance with the manufacturer's specifications
and/or the
American National Standard Institute ANSI B31.1.  (3)  establish bolting
procedures, train employees to utilize those procedures, and ensure their
understanding of the advantages and limitations.    (4) Develop and
implement a
maintenance program for the inspection, maintenance and replacement of
bolts in
conformance with the requirements set forth in Chemical Manufacturing
Association, Mechanical Integrity, Sections 4.4 and 5.0, September 1994.

Ensure that employees follow the industry standard practice of having two
threads
showing above the bolt.  CERTIFICATION AND DOCUMENTATION OF
ABATEMENT REQUIRED.
