mechanical equipment such as
but not limited to pumps, frac tanks, and ventilation equipment during
periodic maintenance on the,J.H.
Caldwell Lease, in Rosharon, TX.
Among other methods, one feasible and acceptable abatement method to
correct the hazard is to establish
and enforce an adequate facility lock out policy such as that stipulated
by ANSI Z244.1 1982, Sections
4 and 5, American National Standard of Minimum Safety Requirements for
Lock out/Tag out of Energy
Sources for Personnel Protection.  The lock out procedure may include, but
is not limited to, the
following essential elements:
1)Hazardous energy control program.
a)energy control procedures for all mechanical equipment.
i.A specific statement of the intended use of the procedure.
ii.Specific procedural steps for shutting down, isolating, blocking and
securing machines or equipment control hazardous energy.
iii.Specific procedural steps for the placement, removal and transfer
of lockout devices.
iv.Specific requirements for testing a machine or equipment to
determine and verify the effectiveness of lockout devices and other
energy control measures.
b)Employee training.
c)Periodic evaluation of the program.
