free from the recognized hazards that were causing or likely to cause
death or serious physical harm to employees in
that:
(a) At this site (29 N and Amer Circle, Madison Heights, VA), Steve Lloyd
& Co., Inc. failed to ensure that the drive
pins used with the concrete forms were guarded to the hazard of
impalement. There were approximately 131 pins
which ranged between 12 to18 inches in height above the ground.
"One feasible and acceptable abatement method, among others, to correct
this hazard is to cap the pins."
