a)Jobsite, on or about 11/8/01: Employees operating number 91 Gehl Skid
Loader, SL6625, with the secondary operator restraint bar broken off.
Among other methods, some feasible and acceptable methods to correct this
hazard is to:
(1) Ensure that the skid loader is in good operating condition and that
all its
safety equipment is correctly installed and utilized;
(2) Ensure that the operator's manual is maintained in the operator's cab;
(3) Ensure that every operator is familiar with the safety equipment to
include its function and purpose; and
(4) Instruct operators to report malfunctions or unsafe conditions prior to
operation.
