Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of a pressure vessel explosion and catastrophic
release of anhydrous
ammonia:
a. Building 6 compressor room on or about 01/24/02: Pressure relief devices
installed for the protection of the main receiver with maximum working
pressure of 250 psi, were not replaced every 5 years. The pressure relief
devices currently in use were installed in 1992.
Among others one feasible abatement method to correct this hazard is to
follow the International Institute of Ammonia Refrigeration (IIAR) bulletin
110, inspection and maintenance of ammonia refrigeration systems. This
document requires that relief devices be replaced every 5 years with
devices
selected in accordance with ANSI/ASHRAE-15
Abatement documentation is required for this citation.
