Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck-by falling materials:
a)Lumber sorting and stacking area, on or about 02/25/02: A 5-ton overhead
hoist used for lifting materials including but not limited to stacks of
lumber
presented a hazard to employees in that the:
1)Hoist was not being inspected on a frequent and periodic basis.
2)Support structure and monorail were not designed and installed by a
qualified person.
3)Hoist system was not load tested prior to use.
ABATEMENT NOTES:
Among other methods, one feasible method of abatement is to:
1) Conduct frequent and periodic inspections of the electric hoist for
wear,
deterioration and malfunction as per ASME B30.16-1987.
2) Request a qualified person to inspect the support structure for the
hoist
system as per ASME B30.16-1987.
3) Load test the hoist system as per ASME B30.16-1987.
b)Lumber sorting and stacking area, on or about 02/25/02: Below the hook
lifting device used for lifting stacks of lumber had no rated load
markings,
and no initial, frequent and periodic inspections were performed.
Among other methods, one feasible method of abatement is to:
Mark the lifting device with the rated load capacity, and conduct frequent
and periodic inspections of the lifting device as per ANSI B30.20-1993.
Abatement documentation must be submitted for these items.
