daily and weekly periodic inspections are performed and recorded per
manufacturers manual pages 10 and 11, also ANSI/ALI ALOIM - 2000,
Chapter 5, Sections 5.1, 5.4, and 5.6.1.

lever operated chain hoist used by lineman/field technicians to tension
suspension strand or cable had throat latches missing from the load and
support hooks.
ABATEMENT NOTE: One feasible means of abatement is to ensure that
throat latches on hooks of manually lever operated chain hoists are
maintained per American National Standard ASME HST-3M-1991, Section
3.4(b).
