a)Vehicle Repair Area, West Bay, on or about 4/24/02: Periodic inspections
(every 3 months) for Benwil model FP-7X, automotive lift, sn.41610,
capacity
7000lbs., were not being done and/or recorded per lift manufacturer's
recommendations and ANSI/ALI ALOIM-2000. Employee was operating a
Benwil model FP-7X, automotive lift, sn. 41610 when wire rope lifting cable
broke. Wire rope was not being serviced or maintained per manufacturer's
manual.
ABATEMENT NOTE: One feasible means of abatement is to ensure that
periodic inspections (every 3 months) are performed and recorded per
manufacturers manual pages 1 and 5, also ANSI/ALI ALOIM-2000, Chapter
5, Section 5.1, 5.4, and 5.6.1.and ensure that wire rope is serviced and
maintained per manufacturer's manual pages 1 and 5.
b)Vehicle Repair Area, Center Bay, on or about 4/24/02: Periodic
inspections
for Autolift model 9000, 2 post, automotive lift, capacity 9000lbs., were
not
being done and/or recorded per lift manufacturer's recommendation and/or
ANSI/ALI ALOIM-2000.
ABATEMENT NOTE: One feasible means of abatement is to ensure that
periodic inspections are performed and recorded per manufacturer's manual
and also ANSI/ALI ALOIM-2000, Chapter 5, Section 5.1, 5.4, and 5.6.1.
Abatement certification must be submitted for this item.
c)Vehicle Repair Area, East Bay, on or about 4/24/02: Periodic inspections
for
Challenger II, 2 post, automotive lift, sn.212417, were not being done
and/or
recorded per lift manufacturer's recommendation and/or ANSI/ALI ALOIM-
2000.
