Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: A CRUSHING HAZARD CREATED BY THE SUDDEN AND UNPREDICTABLE DECENT OF VEHICLES ELEVATED BY TWO POST, SURFACE MOUNTED, ELECTRIC/HYDRAULICALLY OPERATED LIFTS. a)Service area, service bay for Technician #99, on or about 08/09/02: Benwil Industries two post, surface mounted electric/hydraulic automotive lift, serial # B8902064 - mechanical arm locking devices for the front left and right sides and the right rear side were inoperative. b)Service area, service bay for Technician #177, on or about 08/09/02: Benwil Industries two post, surface mounted electric/hydraulic automotive lift, - mechanical arm locking devices for the rear left and right sides were inoperative. c)Service area, service bay for Technician #9, on or about 08/09/02: Benwil Industries two post, surface mounted electric/hydraulic automotive lift, serial # B8902069 - mechanical arm locking devices for the front left side and the right and left rear sides were inoperative. d)Service area, service bay for Technician #105, on or about 08/09/02: Benwil Industries two post, surface mounted electric/hydraulic automotive lift, serial # B8902063 - mechanical arm locking devices for the front left side and the right and left rear sides were inoperative. e)Service area, service bay for Technician #11, on or about 08/09/02: Benwil Industries two post, surface mounted electric/hydraulic automotive lift, serial # B8902070 - mechanical arm locking devices for the front right side and the right and left rear sides were inoperative. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO... 1.INSTALL AND/OR REPAIR MECHANICAL LOCKING DEVICES. 2.INSTRUCT ALL EMPLOYEES IN THE SAFE USE AND OPERATION OF THE AUTOMOTIVE LIFTS AND THEIR SAFETY DEVICES AND ENSURE THEIR ROUTINE USE. 3.ESTABLISH A PERIODIC INSPECTION PROCEDURE AS RECOMMENDED BY THE LIFT MANUFACTURER TO ENSURE THE SAFE OPERATION OF THE LIFTS.4.FOLLOW THE REQUIREMENTS OF ANSI/ALI ALIS-2000, THE ANSI/ALI B153.1 STANDARD AND THE AUTOMOTIVE LIFT INSTITUTE SAFETY MANUAL "LIFTING IT RIGHT". Abatement certification must be submitted for this item.
