not wearing seatbelts provided while operating a Hyster powered industrial
truck, SN: C10868324k-8001, Model: X40XL, and was thrown from the
vehicle during an accident.
ABATEMENT NOTES: Among other methods, one feasible and acceptable
abatement method to correct this hazard is to require employees to wear
seat belts
when operating powered industrial trucks in accordance with ASME
B56.1-1993,
chapter 5.3.19.
Abatement certification must be submitted for this item.
