employee, conditions of employment and a place of employment free
from recognized hazards which caused or were likely to cause death
or serious injury to employees:
Two employees were injured on June 24, 2002, when the scissor
lift they were operating in the elevated position hit a hole
and tipped over onto a concrete surface.  A workplace
inspection had not been performed, which would have identified
and eliminatd holes and obstructions.  Additionally, the
operator had not been provided with a clear view of the path
of travel, which would have assisted the operator in avoidance
of the hole.ance
