Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: A.) Personal fall arrest protection equipment was not inspected periodically (not less than twice annually( by a competent person according to the manufacturer's recommendation, as required by the American National Standard for construction and Demolition Operations requirements for Safety Belts, Harnesses, Lanyards, and Lifeline for Construction and Demolition Use, ANSI A10.14-1991, Section 6.1.2, resulting in the employees using field modified personal fall arrest equipment and lanyards that were visibly damaged. Among other methods, One feasible and acceptable method to correct this hazard is to inspect all personal fall arrest equipment as required by ANSI A10.14-1991, Section 6.1.2, and maintain a written record of those inspections.

Personal fall arrest protection was not visually inspected for defects prior to each use, as required by the American National standard for construction and Demolition Operations- requirements for Safety Belts,Harness, Lanyard, and Lifeline for Construction and demolition Use, ANSI A10.14-1991, Section 6,1,4, resulting in the employees using modified personal fall arrest equipment and lanyards that were visibly damaged. Among other methods, one feasible method to correct this hazard is to inspect all personal fall arrest equipment prior to use as required by ANSI A10.14-1991, Section 6.1.4
