The employer failed to ensure that warehouse employees being elevated by a
forklift were
protected by using an approved work platform.
a.In the warehouse, employees requiring access to the upper levels of a
storage rack
were being lifted on a pallet by a forklift, instead of using an approved
work
platform, exposing them to a fall hazard of up to 12 feet.

personnel.  If

approved work
platform is used to lift personnel with a forklift.
