Nevada Revised Statute 618.375(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: The employer failed to ensure that an employee being elevated by a rough terrain forklift was provided a personnel platform to work from: a.An employee was observed standing on the forks of a rough terrain forklift, 10 feet above a lower level, without a personnel platform. The employee observed was the site foreman. Reference: ANSI Standard ASME B56.6 - 1992 Safety Standard for Rough Terrain Forklift Trucks - Part II Section 5.15.1. If a rough terrain forklift must be used to lift people, the following requirements apply: Provide a personnel platform which complies with the design requirements in Part III of this Standard. One feasible and useful abatement method, among others, would be to ensure that employees being lifted by rough terrain forklifts are provided a personnel platform.
