General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970. a) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees, and other workers at the job-site, were exposed to the hazard of being struck by the falling Hambro Composite Floor System due to unexpected movement. The Hambro Composite Floor System was not properly installed and secured in accordance with the Manufacturers Installation Manual and the Architects Construction Plans. 1) The required two rows of continuous temporary bottom chord bracing were not provided. 2) The Rollbar Angle "RA" was not attached to the walls. 3) The #J18 Hambro Joist that was placed on top of the metal studs was not secured in place. One feasible and acceptable abatement method to correct this hazard is to install the Hambro Composite Floor System in accordance with the Manufacturers Installation Manual and the Architects Construction Plans which require the following construction procedures; 1) The installation of two rows of continuous temporary bottom chord bracing. 2) That the Rollbar Angle "RA" is to be attached to the walls with quarter inch diameter pins, or cut nails, at 2 foot on center. 3) That joists that are placed on top of metal studs are required to be secured in place with 2 #14 TEK screws, or equivalent.
