Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer failed to provide a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees: a. On or about August 22, 2001, at the jobsite located at 1315 West Belmont Avenue in Chicago, Illinois, an employee working from a platform supported by a rough terrain forklift truck was exposed to injuries from falls due to improper elevating of personnel. The proper method is required by the American National Standards Institute's standard for rough terrain forklift trucks. (ANSI B56.6-1992). No abatement certification or documentation is required for this item.
