Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm in that
employees were exposed
to the hazard of falling 25-feet during improper use of a self-propelled
elevating work platform:
a) in that employees had used the elevating platform of the company truck
to extend the
height/reach of an 8-foot mobile ladder stand, as observed on or about
08-23-01.
Additionally, the ladder stand was placed on two stacks of seven pallets
which inturn
were secured to the truck's platform; total height of the ladder stand
platform was more
than 25-feet.
One feasible means of abatement, among others, would be complying with
American
National Standards Institute (ANSI) Standard A92.6-1990, Self-Propelled
Elevating
Work
Platforms, paragraphs 7.11.1 and 8.10.1 prohibiting the use of ladders or
any other
device on the aerial platform for achieving additional height or reach.
Another would
be the use of aerial type lifts designed to elevate employees while inside
a safety basket.
