Lincoln Avenue
in Chicago, Illinois, an employee working on the roof operating a Garlock
hand-powered
material hoist was exposed to injuries due to improper use of
counterweights. The proper
usage of the material hoist requires that permanent counterweights be
used. This information
is provided by the manufacturer in the manufacturer instructions.
Abatement documentation is required for this item in accordance with the
requirements of 29
