Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer failed to provide a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees: a. On or about September 14, 2001, at the workplace located at 1912 North Lincoln Avenue in Chicago, Illinois, an employee working on the roof operating a Garlock hand-powered material hoist was exposed to injuries due to improper use of counterweights. The proper usage of the material hoist requires that permanent counterweights be used. This information is provided by the manufacturer in the manufacturer instructions. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
