recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing hazards created by defective shop made spreader
bars designed for
lifting, in that the brackets for the lifting hooks were not secured nor
had the spreader bars been
load rated to determine lifting capacity.
Among other methods, one feasible and acceptable method would be to load
rate the spreader
bars and adequately attach the brackets for holding the lifting hooks in
place.
(a)At Adams Brothers Produce, Co. - Battery Changing Area - The brackets
for the hooks
on the shop made spreader bar were not secured to prevent the hooks from
slipping out
of the spreader bar.
