were working in
a Terex TS 20 scissor lift with damaged top rails that were 27.5 inches
from the platform
and were observed standing and climbing on the intermediate or mid rail
and top rail of
the platform.
Among other methods, one feasible and acceptable method would have been to
ensure that the
guardrail system on the lift was repaired or replaced and met the
requirements of American
National Standards Institute, ANSI A92.6-1999 Standard for Self Propelled
Elevating Work
Platforms and prevent employees from climbing onto the top and
intermediate rails of the lift.
