Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing injuries from falling loads: (a)On or about 10/30/2002 - The Shaw-Box Motor Driven Trolly Hoist in the Edmonds Area - The operator was exposed to being struck by falling cable reels due to the running wire rope and sheave guards of the hoist being severely damaged. The damage included numerous broken wires, rope distortion from wear, and kinking. Among other methods, one feasible and acceptable method to correct this hazard would include, but would not be limited to: Replace the damaged rope and worn parts of the load block. Incorporate an inspection program in accordance with ASME B30.16-1993 to specifically include wire rope Frequent Inspections set forth in ASME B30.16-2.1.2.
