recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fall hazard of 19 feet:
a.   On or about 4/15/03, employees were lifted by a forklift without the
use of an approved
personnel platform and personal fall arrest system was not utilized.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to comply with the requirements of the American Society of
Mechanical Engieers
documents ASME/ANSI B56.6-1992, Section 5.15, Safety Standards for Rough
Terrain Forklift
Trucks.
